What Do I Need


ENROLLMENT

Everyone is started off with a comprehensive enrollment appointment when enrolling in our Programs that does include: general intake packet, substance use/abuse assessment packet when appropriate and an orientation interview into the program. During the orientation, the level of care is determined in accordance with diagnostic results as to assure that an absolutely personalized treatment plan is created, providing assurance that services and needs match in order to offer attainable and realistic treatment goals.

REQUIREMENTS FOR ENROLLMENT

  • COURT PAPERS
  • REFERRAL (proposition 36 only)
  • PHOTO IDENTIFICATION
  • ENROLLMENT FEES-WHEN APPLICABLE
  • READINESS FOR CHANGE
